- Pass Certificate of Class 10th Exam.
- Pass Certificate of Class 12th Exam.
- Must require NEET UG Mark Sheet of Current Academic Year.
- Aadhaar Card (original and photocopy).
- Need School Leaving Certificate.
- Migration Certificate.
- (Domicile) by the Government of Gujarat.
- Need Scheduled Caste Certificate (if belong to SC category).
- Physical Fitness Certificate (Health Examine).
